Structured Descriptive Assessment

A mainly qualitative but evaluative description of, e.g., a patient or subject that fills in the description under a prescribed set of evaluative headings e.g., indicating degree of antisocial behavior. Note: there is a certain tradition in the social sciences that suggests that the phrase 'evaluative description' is a contradiction in terms, but this is clearly inconsistent with common usage and in fact with scientific usage since it's common for scientists to describe a literature search as inadequate or an experimental design as inelegant.
